This Live activity, Cardiac Disease and Safety in Clinical Research, with a beginning date of October 5, 2011 has been reviewed and is acceptable for up to 15.5 Prescribed credits by the American Academy of Family Physicians. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

MEET AND GREET RECEPTION

Immediately following the CSRC Annual Meeting, there will be a meet and greet reception on October 5 from 6:30-8:30 pm at the Sheraton Washington North in the White Oak room. If you plan on attending, please indicate so during registration.

CANCELLATION POLICY

All meeting cancellation requests must be submitted in writing via e-mail (eva.hill@duke.edu) or by fax (919-668-7114; Attn: Eva Hill).Cancellation Policy: Written notification of cancellations must be received on or before September 30, 2011. Cancellations from October 1 and October 3 will be subject to a $100 charge. Cancellations received after October 3 will not be refunded.In the unlikely event the program is canceled or postponed, we will not be responsible for any travel costs or expenses, including cancellation/change penalties assessed by airlines, travel agencies or hotels.

Driving Directions:

Metro Station Shuttle:

Additional logistical considerations:

Cameras, recordable devices, sharp objects, etc., will not be allowed to pass through FDA security. Laptops, cell phones and jump drives are permitted.
